Practical answers before the first visit.
If you are deciding whether to book, gather imaging, or plan around a busy week, these are the questions patients ask most often.
What insurance plans do you accept?
Send a message through the form on this page with your insurer and plan name, and you will get a straight answer on participation and on whether a referral is needed before your visit.
What should I bring to my first visit?
Bring MRI or CT images if you already have them, the written reports, a list of medications, prior treatment notes if they are easy to access, and a short timeline of your symptoms. If you do not have imaging yet, the office can help determine what is actually needed.
Can I schedule a second-opinion consultation?
Yes. Second opinions are welcome, especially when you have already been told you may need surgery and want a clearer explanation of the diagnosis, timing, and alternatives.
Are virtual visits available?
Virtual visits are available for select follow-ups, imaging reviews, and some second-opinion conversations, particularly for patients outside the Chicago area. Whether a visit can stay virtual depends on the symptoms, imaging, and whether a hands-on exam is still needed.
Where do you see patients?
Consultations are offered across Chicago and the north, west, and south suburbs, with telehealth for patients outside the area. Send a message through the form and you will get the visit details that fit where you are and how soon you need to be seen.
What languages are spoken in the practice?
Visits can be conducted in English, Farsi, and conversational Spanish.

A few specifics help me point you in the right direction: what's hurting, for how long, and anything you've already tried. All inquiries are confidential.
